Insurance Information Submission and Reimbursement Process

Thank you for choosing Elm Counseling for your therapy needs. While we currently do not directly accept insurance payments, we can assist you in seeking reimbursement from your insurance company. Please follow these steps to submit your insurance information:

1. Provide Insurance Information: Please fill out the insurance information form provided to you. Include your insurance company name, policy number, and any other required details.

2. Self-Pay Rates: You will be charged our self-pay rates for therapy services at the time of each session.

3. Claims Submission: Elm Counseling will submit claims through Mentaya company set up to assist with OON benefits.

4. Reimbursement Process: Once the claim is processed by your insurance company, they will reimburse you directly according to your plan's coverage for out-of-network providers. This process can take up to 6 weeks or longer for payment to be reimbursed.

5. Coverage Verification: You may want to contact your insurance company beforehand to verify your out-of-network mental health benefits. This can help you understand your coverage details and any potential reimbursement amounts.
